Reconstructing a new nose for Lò Thị Lệ, a girl who faced severe necrosis that caused her nose to nearly 'disappear,' became a tough challenge for the medical team at Kangnam Cosmetic Hospital. Lo Thi Le (borned in 2001, Sơn La) faced difficulties from the age of 8 months. The pressure from the judgmental stares of those around her prevented Le from living like a normal girl. She was shunned and mocked by her peers, which led her to gradually lose confidence and eventually drop out of school because she could no longer bear the emotional strain. Le’s father, although heartbroken for his daughter, could not afford the treatment costs due to the family’s difficult financial situation. However, he never stopped wishing that his daughter could receive treatment, return to school, and find happiness again.
When the team at Kangnam Cosmetic Hospital took on Lo Thi Le’s case, they evaluated it as a complex and challenging nasal surgery. Le’s nasal area had suffered severe damage—her nasal tip, nasal alar, and nasal septum were completely gone, and the surrounding structure was heavily scarred with contractures. It was nearly impossible to use the local skin flap, so they had to resort to using a distant skin flap.
"To reconstruct Le’s nose, various techniques had to be applied, including synchronizing the nasal structural framework and using skin flaps to cover both the internal and external mucous membranes of the nose, shaping a complete nose. 'This is a challenging surgery that requires multiple stages,' said Dr. Richard Huy, head of the 'Journey of Transformation' medical team.
Stage 1: Skin Expansion and Scar Revision
The first challenge the doctors faced was the process of inserting a skin expander. The procedure of injecting water into the expander was delayed because of the time required to treat infections, which prolonged the surgery. The purpose of placing the skin expander on the forehead was to use an autologous skin flap to cover the nasal defect.
Stage 2: Flap Transfer and Nasal Reconstruction
The medical team decided to use rib cartilage to reconstruct the entire nasal tip, nasal alar, and nasal septum. At the same time, they transferred the flap to shape the nose. This process required high-level technical expertise from the doctors. With years of experience handling complex defect cases, the doctors successfully performed the surgery beyond expectations."

Stage 3: Thinning the nasal flap + Lifting the nasal tip with ear cartilage + Nanofat injection
The medical team focused on thinning the already-shaped nasal flap to enhance the structure of the nasal tip, using ear cartilage to create a softer and more natural nasal tip. Additionally, the nasal area underwent a Nano Fat injection to regenerate soft tissue and fill in any deficiencies.
Stage 4: Correcting scar contractures and second thinning of the flap
In this stage, the medical team concentrated on correcting severe scar contractures around the nose, combined with the second thinning of the nasal flap to rectify any irregularities and create uniformity in the nose’s shape.
Stage 5: Trimming the nasal base, third thinning of the flap, and completion
The doctors performed trimming of the nasal base to refine the shape of the nose, along with the third thinning of the nasal flap to complete the surgical outcome, ensuring the nose has a more natural and balanced appearance.
These complex surgeries, lasting several hours, required intense focus from the entire team. Every small detail had to be carefully considered, from choosing the right reconstructive materials to precisely shaping the soft tissue and bones. After much effort, the medical team successfully reconstructed Lo Thi Le's nose, giving her a new appearance and hope for a better life.
